It includes information such as name, address, phone number, date of birth, gender, marital status, social insurance number, personal health number, date of accident, employer's name, occupation, injury type, medical treatment record, medical history, vocational assessments, investigation reports, appeals decisions, earnings information and compensation payment details. |
