SCOPE AND CONTENT: The series consists of certificates (known as Form 4) issued by the Office of the Chief Medical Examiner under the Fatalities Inquiries Act for the cremation, dissection or shipment of a body outside the province.. The certificates contain the individual's name, age and gender, as well as details about the death, specifically the cause, date and place. Other information on the certificate includes the location of the body, and the date the certificate was issued. In some cases, a copy of the physician's certificate of death may be attached..
GENERAL NOTE: Certificates from Edmonton for 1984-1992 and from 1993 for individuals with the last names A-M were inadvertently destroyed by the Office of the Chief Medical Examiner.
